This form is to be printed and filled out or submitted electronically to resident school superintendents by October 1 of each year, or within 15 days of withdrawing a child from public school in order to homeschool.

This form is to be printed and filled out or submitted electronically to the resident school superintendent by October 1 of each year after the instructor has provided the same district with a Full Report.
Help for Filling out the Compulsory Report Form
Nothing in Minnesota law specifies exactly how non-public educators are to make their annual report only that it is due by October 1 each year and certain information is to be communicated. You are free to write a letter of your own, use this MACHE designed reporting form, or some other device.

Learn why parents removed their children from public schools to educate them at home and how these pioneers fought for that right before local courts, district courts, the Minnesota Supreme Court, and in hearings before both the Minnesota House and Senate. Witness the birth of MÂCHÉ and its thirty years of growth and influence. Be informed, encouraged, and inspired by these mostly ordinary yet courageous people.
Given Hoffman is a homeschool graduate and full-time writer. Eileen Hoffman is a current member of the MÂCHÉ Board of Directors and retired homeschool mother of five. Mother and daughter reside in the cold wonderland of Northern Minnesota and enjoy sitting in front of the fire with hot chocolate and computers, writing books, articles, and blog posts.
