Pop Warner Scholar Forms
Select the “Home School Form.” I spoke to Josh Pruce, National Director of Scholastic and Media Relations for Pop Warner, and he explained to me that all that is needed is a brief progress report on what your child has been doing and how well they’ve been doing it from the start of your fall homeschool program until you file the form between September 15 and October 15. If your child is enrolled in any homeschool program then provide an official report. If not, then an informal report created by a parent is fine. You may contact Mr. Pruce at 215-752-2691 x23 if you need clarification.
